- Health Certificates – Download and Fill In the Health Certificate (private of public doctors).
Note: For public doctors, apart from their personal stamp, the official stamp of the Hospital is also required. If you choose to visit a private doctor, you will need to verify the copy of the official certificate in a lawyer.
- Photocopy of Personal ID
- Face Picture (Taken by a mobile device, full face, in white background. It will be uploaded in the Online Registration Form) EXAMPLE OF A CORRECT PICTURE TO UPLOAD!
