I declare responsibly that the information stated in this registration application is true.
At my own risk and knowing the consequences of the law and the sanctions of article 8 of the Responsible Declaration Law 1599/1986, which are provided from the provisions of par. 6 of article 22 of Law 1599/1986, I declare responsibly that: on the day of my application I am over 18 years old, I will follow the regulations of the Hellenic Lifeguard Academy, I am able to swim without a cruising aid, I know how to swim underwater and I do not suffer from physical, mental or emotional illness and I have not had a serious health problem in the past.
Generally i am in a good state of health, i do not use or have used drugs, acohol or hallucinogenic substances and i am able to participate in the activities & trainings of the Hellenic Lifeguard Academy, which I have chosen acting with prudence and will, so as not to cause harm to myself and other participants
I wish to participate, knowing the potential risk, accepting the possibility of an accident and taking on the consequences for which the organizing company "ENAK Lifeguard School - Hellenic Lifeguard Academy", legally represented by Mr. Giovanidis Nikolaos bears no civil or criminal liability, and state that i will not claim anything in the event of an accident or injury.
I provide my explicit and unconditional consent to the retention and processing of my personal data, such as indications (name, address, profession, email, telephones, etc., photos and videos) for use by the Hellenic Lifeguard Academy and its authorized personnel, in the context of its statutory purposes and its educational activity, and under the conditions provided by Greek Law 679/2016 as in force.
